The Mid-Eastern Chapter of the Pennsylvania Society of Health-System Pharmacists, Boehringer Ingelheim, and Amedco is proud to invite you to a one hour promotional presentation on Transitional Care and a one hour CE on International Medication Safety from ISMP on Thursday, April 19th, 2018!

1. List the top medication safety priorities at ISMP for 2018
2. Compare and contrast medication safety in the United States to that which is available in Cameroon
3. Restate the goal of the World Health Organization's Global Patient Safety Challenge on Medication Safety 
4. Relate root causes of a fatal medication error occurring in New Zealand to similar systems in the United States
5. Summarize strategies to prevent a similar medication error from occurring in United States health systems

Amedco is an approved by the Accrediation Council on Pharmacy Education (ACPE) as a provider of continuing pharmaceutical education that complies with the Criteria for Quality for continuing pharmaceutical education programming. This program ACPE Program ID is acceptable for 1 hour (0.1 CEU) of continuing education credit in states that recognize ACPE Providers. Statements of completion of continuing pharmaceutical education will indicate date, hours and CEUs of the program based on the participant’s attendance. After the live CE, attendees must complete a continuing pharmaceutical education evaluation on-line to receive continuing education credit
